Significant Event Analysis - Template

Significant event analysis  (SEA) is a quick way of getting to those learning needs in the "hidden" box of Johari's window. Crudely speaking it is a way of learning from our mistakes. The Registrar, trainer or other member of the practice team raises an event as significant such as a missed diagnosis, death of a patient or a practice complaint. All the parties involved discuss why the event is considered significant and determine the facts of the case. The important issues raised by the event are teased out. Those things that went well are highlighted together with those that went badly. Feasible areas for improvement are identified and an action plan drawn up. The SEA can then be written up and filed in the learning log or Personal Development Plan.The key features of success of SEA are (i) that it should be a positive experience for all involved (ii) that it should result in some improvement in patient care (iii) that it is about improvement and development - not blame.

Clinical Care

  • 5 significant event audits in the 5 year cycle. GP must be directly involved and the event must demonstrate areas for improvement, reflection and implementation of change.
  • 2 completed audit Cycles i.e. audit change, and re-audit to demonstrate improvement.


Good Medical Practice

  • Meet criteria set out by relevant faculty for CPD (i.e. 50 RCGP credits a year)
  • This will include reflection on personal learning


Patients

  • Result of latest patient’s survey, with reflection. (Must be 2 within past 5 years and relate to the INDIVIDUAL practice of the appraisee. If over a year ago a record of subsequent action must be included)
  • Reflection on each complaint or declaration of no complaints

 

Colleagues

  • Result of latest Multi Source Feedback, with reflection. (Must be 2 within past 5 years. If over a year ago a record of subsequent action must be included)


Teaching / Research / Management

  • Declaration of Professional Roles
  • Any performance management evidence from these roles
  • Evidence that you are qualified to undertake this role and are up to date and can demonstrate that you are fit to practice in this role

 

Probity

  • Standard probity declaration
  • Self declaration regarding any performance procedures by PCT, GMC, National ClinicalAssessment Service, criminal proceedings


Health
